Privacy Policy

The Regen Providers Network, LLC ("The Regen Providers," "we," "us," or "our") respects the privacy of healthcare professionals who interact with this website and submit applications through it. This Privacy Policy describes the categories of information we collect, how we use that information, and the choices available to you. By using this website or submitting an application, you consent to the practices described below.

1. Information We Collect

We collect information that you provide directly to us when you complete the provider application form, including your full name, email address, phone number, professional credential type (MD, DO, APRN, CRNA, or PA), Florida medical or nursing license number, National Provider Identifier (NPI), Drug Enforcement Administration (DEA) registration if applicable, malpractice insurance carrier, years of clinical experience, county or coverage preference, and modality preferences (in-home, in-office, or hybrid). We may also collect attestations and acknowledgments you provide regarding your independent contractor status, federal and state regulatory awareness, and consent to be contacted.

We automatically collect limited technical information through cookies and similar technologies, including IP address, browser type, device type, referring URL, pages visited, and timestamps. See our Cookies Policy for details.

2. How We Use Your Information

We use the information you submit to evaluate your application for engagement as an independent contractor providing in-home or in-office regenerative care services in Florida; to verify professional credentials, licensure, and insurance status; to communicate with you about the application process and onboarding; to operate, secure, and improve this website; and to comply with applicable legal obligations. We do not sell applicant information.

3. Sharing of Information

We share information only with: (a) our Medical Director and clinical leadership for credential review; (b) third-party service providers who help us operate the application pipeline (including our customer relationship management platform, email and SMS delivery providers, and credential-verification services), each bound by confidentiality obligations; (c) governmental, regulatory, or licensing bodies when required by law or in response to valid legal process; and (d) successors-in-interest in connection with a merger, acquisition, or sale of assets, where the recipient agrees to honor this Policy.

4. Data Retention

We retain applicant information for as long as reasonably necessary to evaluate your candidacy, fulfill onboarding obligations if you are engaged, and meet recordkeeping requirements under applicable Florida and federal law. You may request deletion of your application record by contacting [email protected], subject to legal retention obligations.

5. Security

We implement administrative, physical, and technical safeguards designed to protect applicant information from unauthorized access, disclosure, or loss. No method of electronic transmission or storage is one hundred percent secure, however, and we cannot guarantee absolute security.

6. Your Rights

Depending on your state of residence, you may have rights to access, correct, delete, or restrict our processing of your personal information. California residents have additional rights under the California Consumer Privacy Act (CCPA), as amended. To exercise any of these rights, contact us at [email protected]. We will verify your identity before responding.

7. HIPAA Notice

Information collected through this website at the application stage is provider-side professional information and is not protected health information under HIPAA. Once you are engaged and begin seeing patients on our behalf, you will be required to execute a Business Associate Agreement and follow our HIPAA-compliant clinical documentation platform.

8. Changes to This Policy

We may update this Privacy Policy from time to time. We will post the updated version on this page and update the "Effective" date above. Material changes will be communicated to active applicants and engaged contractors by email.
